This one-time assessment offers an invaluable deep dive consultation where you learn about the unique imbalances that are impacting your symptoms. This appointment is necessary to establish care at Dream Functional Medicine and is only available for Colorado residents 18 years and older.

A Non-Refundable Purchase *

I acknowledge that the Foundational Assessment is a non-refundable service 120 days after the purchase date. It is the patient’s responsibility to complete onboarding paperwork and labs in a timely manner to allow for lab processing and appointment scheduling. If labs, paperwork, and appointment scheduling are not completed within 120 days, the FA becomes nonrefundable.
